and distribute it to students to take home to parents.
Print off this form, THE ORDER FORM, and distribute it to students to take home to parents.
The BOOK SELECTION SHEET should be included with this form.
Have students return their completed form to their classroom teacher.
Print this form, THE SUMMARY SHEET, for teachers to tabulate the total books
ordered and funds collected for their class.
Once all students orders have been received and recorded by their teachers,
print and complete this final FAX FORM to tally your school's order
OR complete the secure online form.
